Resumption of Care Assessment
The Resumption of Care Assessment creates a Resumption of Care order. The Resumption of Care order contains the Plan of Care for the remainder of the existing certification period for the patient. For example, the patient is hospitalized over 24 hours for reasons other than testing in the middle of of the certification period. The resumption assessment is completed when the patient returns home.
If the Resumption of Care Assessment is completed in the last five days of a certification period, it also creates a Recertification Plan of Care Order.
Do one of the following:
From the patient chart, click Case Details, click Charting, click Add and select Med-Pass Resumption of Care (C2).
From the Care At Home mobile app, tap to start patient appointment.
To enable the Resumption of Care order, hover over Administration, click Care Plan, select Plan of Care (Resumption of Care), and click Accept.
