Overview of Start of Care / Resumption of Care Assessment 

The Outcome and Assessment Information Set (OASIS) is a group of standard data elements. The OASIS contains data elements developed for measuring patient outcomes for the purpose of performance improvement in Home Health Care. The OASIS is contained within the SOC / Resumption of Care, and Re-certification comprehensive assessments.

The SOC assessment, as well as many other documents, are located within the charting library. You can find the charting library located under Care Details. Within the CAH app, based on the type of visit scheduled the SOC or assigned visit type pushes into the appointment. You can add additional documentation to the appointment by tapping Ad Hoc Documentation and tapping on the document you want to add to the visit.

From the SOC Assessment, a large amount of patient data is collected, which in turn formulates a HIPPS code, for reimbursement of the entire 60 day episode. Services, functional and clinical pieces, along with the CBSA code and episode timing, all come together to create the HIPPS code.

For any of the comprehensive assessments, information located within the client chart before the assessment being initiated pulls into the assessment. If new information is added, for example with the initiation of an resumption of care assessment, the new information pushes back to the client chart after the assessment is saved and signed.

Within the SOC/Resumption of Care Assessment, you can add services and the visit dates for the certification period. The services and date frequency is pushed to the 485 after the assessment is signed, to Locator 21. The care plan is also initiated within the SOC/Resumption of Care Assessment. All orders, goals, and outcomes push to Locator 21 and 22 after the assessment is signed. 

After signing an assessment, you can use an OASIS Scrubber. The Oasis Scrubber validates all OASIS/assessment information and consistency of data. The clinician can navigate through the validation process to review and update any assessment responses. You can scrub your OASIS data within the web application, but not within the CAH app. 

Within Case Details, the OASIS section of the client record is where the Home Health Care Tracking Sheet is located. Episodic information such as the SOC dates, HIPPS Code, HHRG codes, CBSA Code and projected payment can be reviewed in billing details. When the OASIS document is reviewed and updated, an export file is created that is later sent for submission. 

After the OASIS assessment is completed, the valid certification period shows the HHRG value of the assessment within the Client Header. 

  • After you complete an OASIS assessment, you should validate or scrub your data by clicking OASIS Scrubber (if available).

  • The agency can select their OASIS Scrubber vendor. If you do not have an Oasis Scrubber, the OASIS Scrubber tab is hidden.
