Managing MED-PASS Therapy Evaluations
You can complete MED-PASS Therapy Evaluations when the Start of Care (SOC) has already been completed by the RN. After the therapy evaluation is completed, information contained within the evaluation flows to the open 485, services, and care plan items, locator 21 and 22 of the 485.
The following evaluations, when completed, push information to the 485:
MED-PASS Physical Therapy Evaluation
MED-PASS Occupational Therapy Evaluation
Med-PASS Speech Language Pathology Evaluation
If the Home Health episode is a therapy only case and the RN isn't involved in the SOC, the therapist completes the MED-PASS Therapy Start of Care/Resumption of Care assessment.
