Continuity Of Care Document Overview
The Continuity of Care Document (CCD) is located within the client header of the patient chart in the Home Care Home Health Care. The CCD can be used to share information regarding patient details. For example, transfer to a hospital or outside provider. The CCD is created automatically from the content located in the patient record. You cannot edit the CCD. If items are incorrect, you can update the patient record and the CCD automatically updates.
The CCD is available in XML and PDF formats.
