Client Header Overview

The Client Header appears in all client tabs in the web application. It is an area where pertinent client-related information appears.

The Client Header shows the following information in the collapsed view including:

  • Name and (Client ID #)

  • Date of Birth and Age

  • Address

  • Phone Number

  • Code Status

  • Allergies

  • Emergency Priority

  • Export CCD

  • Case #

  • SOC (Start of Care) Date

  • Primary Physician phone and fax numbers

  • Branch

  • Case Manager (CM) – if assigned

  • Backup Case Manager (CM) – if assigned

  • Human Resource Generalist (HRG) – if assigned

  • Current Certification Period

  • Certification Period HHRG

From the Client Header, you can also:

  • Add Communication Note

  • Export an XML of the Continuing Care Document (CCD)

  • Export a PDF of the Continuing Care Document (CCD)

  • Switch case types if there are multiple case types for the client.
