OASIS Correction Tools - Correct

You can correct an OASIS record that needs to be updated. The correct option can be used to submit an updated record to CMS for a previously accepted OASIS. The correct option should be used when a non-key field item needs to be updated or changed. An accepted record can be updated multiple times. The number of corrections in the chain is indicated in the # column of the OASIS table. The original number and corrections count up from 0. 

  1. Within the clients chart, click Case Details. Click Oasis and then click Correct

  2. Click Correct next to the OASIS record to be updated. A new OASIS record is generated in an incomplete status. Within the OASIS table, in the # column, you can see a 01 for the first correction.

  3. Make changes as needed and mark record Reviewed.

  4. Generate the OASIS Export Report for re-submission.

  • Not all M00 items can be updated with a correction. Certain fields are considered key fields and must be changed with an Inactivation.

    • Key Fields:

      • Patient Identifiers:

        • M0040 - Patient name.

        • M0040 - Patient first name.

        • M0064 - Patient social security number.

        • M0066 - Patient date of birth.

        • M0069 - Patient gender.

      • HHA Identifiers:

        • HHA AGENCY - Unique Agency ID code.

      • Assessment Event Identifiers:

        • M0100 - Reason for completing assessment.

        • M0090 - Date assessment information completed.

          This is a key field only on recertification of follow-up assessments where RFA = 04 or 05.

        • M0030 - SOC Date. This is a key field only on ROC assessments where RFA = 03.

        • M0032 - ROC Date. This is a key field only on ROC assessments where RFA = 03.

        • M0906 - Discharge, transfer, death date. This is a key field only on transfer to inpatient facility assessments where RFA = 06 or 07, death at home assessments where RFA = 08 and discharge assessments where RFA = 09 or 10.

  • All OASIS corrections should have a supporting Oasis Correction Form in the client chart indicating changes have been made. 

  • The type of OASIS assessment pulls from M0100 within the OASIS assessment:
