Plan of Care/485 Overview

When you complete, save, and sign the comprehensive assessment (Start of Care/Resumption of Care or Re-certification Assessment), the Plan of Care/485 is automatically created with the details from the assessment populated into the appropriate Locator Fields within the 485. The 485 defaults to a status of open.

For the open 485, you can view all current active services from the Care Plan for the certification on the order. Changes made to services on the Care plan flow through to the open 485. After the 485 is sent to the physician for signature, services freeze and are no longer updated from the Care Plan.

Locator 21 contains treatment orders and Locator 22 contains goals and outcomes.

After the Plan of Care/485 is reviewed for accuracy, you can print the 485 and send to the physician for signature. When the Plan of Care/485 is signed by the physician, the order status is updated to signed and the document can no longer be changed.

Order Statuses: 

  • Open - When an order is created, it has a status of Open and the order can be edited in this status. 

  • Sent - When the order is sent to the physician for signature, you can edit the order and change the status to Sent. The order can still be edited in the Sent status, however procedures must be in place to ensure the revised order is sent to the physician for signature.

  • Signed - On receipt of a signed order, edit the order and change the status to Signed and enter the corresponding date. The order cannot be edited in the signed status. The edit link under actions is replaced with view when the order status is changed to signed.

  • Rejected - If an order has the status of rejected, the order is considered inactive and cannot be edited.

  • You can create a blank Plan of Care/485 by accessing Case Details and selecting Plan of Care, however this is not the intended workflow. The intended workflow is to create an assessment from a single point of entry for information within the application.
